1. In signing this form, I hereby state that the information included in this form is correct and give permission for my child to participate in the activity described on this form.
2. I understand that my child will be under the supervision of the Archdiocese of St. Louis staff and volunteers.
3. I recognize that there are risks inherent in participation in any activity and agree to hold the Archdiocese of St. Louis, its affiliates and its employees, volunteers and agents, harmless from any injury to my child or damage to or loss of personal property of my child not caused by the negligence or misconduct of the Archdiocese of St. Louis, its affiliates and its employees, volunteers and agents.
4. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child may be exposed to, or infected by COVID-19 by participation in the activity described on this form during the above dates. On behalf of myself and my child, I hereby release, covenant not to sue, discharge and hold harmless the Archdiocese of St. Louis, its affiliates and its employees, volunteers and agents for any claim for injuries and damages, costs or expenses arising out of or relating to COVID-19 as a result of participation in the activity described on this form.
5. Located on the Campus of Missouri State University are scooters for rent and an outdoor, unsupervised rock-climbing wall. Neither the Archdiocese of St. Louis nor the Conference condone the rental of motorized scooters or climbing of the wall which may be found on the campus where the Conference is held. Any such rental and climbing is discouraged and the below signed party expressly agrees that the Archdiocese of St. Louis, the Conference, their employees, agents and volunteers shall not be responsible for injuries and damages caused by any such rental of scooters or climbing of the rock wall by participant.
6. In the case of a medical emergency, I understand that every effort will be made to contact me, but in the event that I cannot be reached, I hereby give permission for my child to be evaluated, diagnosed and treated in accordance with standard medical practice by licensed medical personnel.
7. I hereby give permission to the Archdiocese of St. Louis to use any photographs or video footage taken of my child in print and on their website for promotional purposes.
8. I understand that for all Youth Ministry activities there is a zero tolerance policy for the use of any mood altering chemicals (including alcohol, marijuana, CDB, THC and illegal drugs), foul language, threats or any type of abuse, inappropriate physical contact, and possessing a firearm, destructive device, or other dangerous weapon. I agree to follow this policy.
