Terms and Conditions for Youth Participants

1. In signing this form, I hereby state that the information included in this form is correct and give permission for my child to participate in the activity described on this form.

2. I understand that my child will be under the supervision of the Archdiocese of St. Louis staff and volunteers.

3. I recognize that there are risks inherent in participation in any activity and agree to hold the Archdiocese of St. Louis, its affiliates and its employees, volunteers and agents, harmless from any injury to my child or damage to or loss of personal property of my child not caused by the negligence or misconduct of the Archdiocese of St. Louis, its affiliates and its employees, volunteers and agents.

4. I acknowledge the contagious nature of COVID-19 and voluntarily assume the risk that my child may be exposed to, or infected by COVID-19 by participation in the activity described on this form during the above dates. On behalf of myself and my child, I hereby release, covenant not to sue, discharge and hold harmless the Archdiocese of St. Louis, its affiliates and its employees, volunteers and agents for any claim for injuries and damages, costs or expenses arising out of or relating to COVID-19 as a result of participation in the activity described on this form.

5. In the case of a medical emergency, I understand that every effort will be made to contact me, but in the event that I cannot be reached, I hereby give permission for my child to be evaluated, diagnosed and treated in accordance with standard medical practice by licensed medical personnel.

6. I hereby give permission to the Archdiocese of St. Louis to use any photographs or video footage taken of my child in print and on their website for promotional purposes.

7. I understand that for all Youth Ministry activities there is a zero tolerance policy for the use of any mood altering chemicals (including alcohol and illegal drugs), foul language, threats or any type of abuse, inappropriate physical contact, and possessing a firearm, destructive device, or other dangerous weapon. I agree to follow this policy.

COVID-19 Terms and Conditions for Youth Participants
I acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing social distancing.

I further acknowledge that the Archdiocese of St. Louis has put in place preventative measures to reduce the spread of the Coronavirus/COVID-19.

I further acknowledge that the Archdiocese of St. Louis and Franciscan University of Steubenville cannot guarantee that my child/dependent will not become infected with the Coronavirus/COVID-19. I understand that the risk of becoming exposed to and/or infected by the Coronavirus/COVID-19 may result from the actions, omissions, or negligence of my child/dependent and others, including, but not limited to, staff and other participants.

I voluntarily seek services for my child/dependent provided by the Archdiocese of St. Louis and acknowledge the contagious nature of the Coronavirus/COVID-19. I acknowledge that my child/dependent must comply with all set procedures to reduce the spread while attending the Steubenville STL Mid-America Conferences 2021.

I attest that at the time of the Conference:
- My child/dependent will not attend if they are experiencing any symptom of illness such as cough, shortness of breath or difficulty breathing, fever, chills, muscle pain, headache, sore throat, or new loss of taste or smell.
- My child/dependent will not travel internationally within 14 days of the Conference
- My child/dependent will not travel to a highly impacted area within the United States of America within 14 days of the Conference.
- My child/dependent will not attend if they believe they have been exposed to someone with a suspected and/or confirmed case of the Coronavirus/COVID-19.
- My child/dependent will not attend if they or a member of their household is awaiting results from a Coronavirus/COVID-19 test
- My child/dependent will not attend if they have been diagnosed with Coronavirus/COVID-19 and not yet cleared as noncontagious by state or local public health authorities.
- My child/dependent will be following all CDC recommended guidelines as much as possible and limiting their exposure to the Coronavirus/COVID-19.
- My child/dependent agrees to follow all guidelines put forward by the Steubenville STL Mid-America conferences at the time of the event.
I hereby release and agree to hold the Archdiocese of St. Louis and Franciscan University of Steubenville from, and waive on behalf of myself, my heirs, and any personal representatives any and all causes of action, claims, demands, damages, costs, expenses and compensation for damage or loss to myself and/or property that may be caused by any act, or failure to act of the Archdiocese of St. Louis, or that may otherwise arise in any way in connection with any services received from the Archdiocese of St. Louis.

I understand that this release discharges the Archdiocese of St. Louis and Franciscan University of Steubenville from any liability or claim that I, my heirs, or any personal representatives may have against the Archdiocese of St. Louis and with respect to any bodily injury, illness, death, medical treatment, or property damage that may arise from, or in connection to, any services received from the Archdiocese of St. Louis. This liability waiver and release extends to the Archdiocese of St. Louis, its officers, agents and employees.

2. I understand that I will be responsible for the supervision of minors from the parish/group with which I am attending. I agree to follow all additional instructions from the Archdiocesan staff regarding said supervision.

3. In the event that I am not coherent or conscious, I hereby grant the staff, volunteers or agents of the Archdiocese of St. Louis permission to act on my behalf in seeking emergency medical treatment for myself in the event that such medical treatment is deemed necessary.

4. I agree to accept any and all financial responsibility as a result of emergency medical treatment.
